HOW CAN I ACHIEVE HIGH CHEEKBONES ?

Last updated : 7 July 2026

High, well-defined cheekbones are among today's most valued facial aesthetic features. They give the face harmonious structure, a youthful impression and an ability to catch the light that enhances expressions.
But can you really achieve “high cheekbones” if you do not have them naturally ? Is it a matter of genetics, or are there medical, aesthetic — or even natural — ways to achieve this sought-after effect without surgery ?
In this article, we explore the causes of limited malar prominence, effective and safe medical solutions to enhance the cheekbones, and the limitations of often idealised natural methods.

What is a high cheekbone and why is it considered a beauty criterion ?

Before discussing treatment, it is essential to define what is meant by a “high cheekbone”. This is not simply an anatomical landmark, but primarily a visual perception within facial balance.
A cheekbone is considered “high” when it naturally follows the line of the zygomatic bone, just above the nasolabial fold, catches light on the upper cheek in line with the eyes, and creates a smooth transition between the lower eyelid, cheek and temple. This alignment creates a slightly lifted effect in the middle third of the face.
High cheekbones are most often associated with well-defined bone structure, but also depend on good fat distribution, firm skin and an absence of skin laxity.

Why do some people not naturally have high cheekbones ?

Not having high cheekbones is not a flaw : it is a natural morphological variation.
Several factors may explain limited prominence or a low cheekbone position :

  • A less prominent zygomatic bone structure, which naturally limits midface projection.
  • An accumulation of subcutaneous fat in the lower cheek, which visually weighs down the central face.
  • Progressive age-related skin laxity, causing the malar fat pad to descend.
  • Facial morphology : cheekbones are naturally less prominent in round, oval or triangular faces.
  • Genetic or ethnic characteristics influencing volume distribution, bone structure and the overall balance of facial proportions.

In some cases, cheekbones were clearly defined in youth but became less visible over time because of deep fat loss and tissue sagging.
Restoring cheekbone definition then involves replacing subtle volume while respecting each face's natural proportions and unique architecture.

Why do high cheekbones make the face look younger and more harmonious ?

For centuries, high cheekbones have embodied a beauty ideal often associated with youth, elegance and distinctive features. Beyond this widely publicised aesthetic value, cheekbones play a fundamental role in facial structure, radiance and the emotional impression the face conveys. Aesthetic medical treatment requires precision, judgement and anatomical expertise to avoid common pitfalls such as a frozen or swollen appearance.

How can hyaluronic acid injections create high cheekbones ?

Hyaluronic acid injections remain the most precise, controlled and reversible method for reshaping or enhancing the cheekbones. Performed in the medical practice, this treatment increases zygomatic projection with great accuracy, restores lost midface volume and creates a subtle lifting effect without surgery or anaesthesia.
The product is injected deeply, directly against the bone, to reproduce natural cheekbone volume. Depending on facial morphology, one to two syringes are enough to achieve a harmonious result. The result appears immediately after the session, then improves over ten to fifteen days as the product integrates harmoniously into the tissues.
Injections offer many advantages : the procedure is quick, requires no social downtime, and the result remains adjustable and reversible if necessary. With appropriate quantities and correct technique, they provide a natural result in perfect harmony with the face.
Results last an average of ten to twelve months, varying with the product used and the patient's metabolism. This approach suits patients seeking to subtly restore facial structure and freshness without changing their expressions or undergoing an invasive procedure.

Why do some cheekbone injections produce an artificial result ?

The objective of aesthetic cheekbone treatment is to restore or enhance natural prominence without changing facial identity. However, some procedures can produce an artificial or disproportionate result.
Several causes may explain these unwanted effects.

  • Excessive injected volume : using too much hyaluronic acid can make the face appear swollen and frozen, losing its elegance. This commonly occurs when trying to correct several areas in one session.
  • Incorrect placement : if the product is injected too low or too far forward rather than positioned towards the upper and rear part of the cheekbone, the face looks heavy and unnatural, without the desired lifting effect.
  • An unsuitable product : hyaluronic acid products do not all have the same properties. Using an overly dense or highly cross-linked gel in a mobile area can restrict facial expressiveness and create a frozen result.
  • Insufficient understanding of dynamic contours : because the face is constantly moving, an inappropriate injection can create visible distortions during expressions, making a prior dynamic assessment essential.

Can you achieve high cheekbones naturally without injections or surgery ?

Many online sources suggest that high cheekbones can be achieved “naturally” through facial exercises, face yoga or massage. Although these methods may help tone the skin and improve circulation, they cannot change bone structure or add genuine volume.
Face yoga may stimulate certain smiling or chewing muscles, improve skin tone and slightly slim the cheek area through drainage. However, it creates no volume, and its effect is limited to very subtle surface tightening that depends on consistent practice.
Massage can reduce swelling in a lower face weighed down by fluid retention, but does not actually raise the cheekbone.
Natural methods can therefore be useful complements, but cannot replace a medical procedure when the objective is significant morphological change.

Why is a medical assessment essential before enhancing the cheekbones ?

Achieving high cheekbones is not a standardised procedure. The result depends closely on the patient's facial morphology, the proportions between forehead, cheekbones and chin, the amount of fat in the lower cheek, and skin quality — including thickness, firmness and elasticity.
A precise assessment determines whether treating the cheekbones alone is appropriate or whether correcting several areas, such as the temples, nasolabial fold or facial contours, is preferable. This comprehensive analysis is essential to ensure a harmonious, natural and balanced result.
It helps avoid common pitfalls such as excess localised volume causing a frozen or swollen appearance, disrupted facial proportions or correction that fails to respect the patient's facial identity.

Are cheekbone implants an alternative to hyaluronic acid injections ?

In very specific cases, particularly when the zygomatic bone is completely flat or underdeveloped, and for patients unwilling to accept temporary results, surgical malar implants may be considered. Performed under anaesthesia, this procedure places a silicone or Medpor implant directly on the zygomatic bone to recreate lasting structural prominence.
This solution is invasive, requires postoperative recovery and is permanent. Nevertheless, it may suit certain extreme cases of constitutional facial hollowing when non-surgical solutions prove insufficient.
However, this is now rarely proposed as a first-line option. In almost all cases, hyaluronic acid injections provide a natural, adjustable result perfectly suited to facial morphology without major intervention.

Are high cheekbones a universal beauty ideal ?

High cheekbones have long been valued. In many civilisations, this facial prominence has been perceived as a marker of vitality, nobility or authority. In Chinese tradition, a well-structured face signifies good fortune and longevity. In sub-Saharan Africa, prominent cheekbones are often considered a sign of health and fertility. In the West, they became especially prominent from the twentieth century with the rise of cinema, fashion and photography, which celebrate angular, radiant faces.
In the digital age, social media and filters further reinforce this model. Sculpted faces with accentuated symmetry have become an implicit beauty norm, particularly in artistic and media circles. This development has generated growing demand for cheekbone correction or enhancement.
However, it is essential to remember that beauty is not universal. The doctor's role is to help each patient enhance their individual features without replicating a standard.
