The muscular origin of frown lines
Frown lines result from repeated muscular overactivity rather than age-related skin laxity. They are dynamic expression lines. They develop through regular contractions in the glabella — the area between the eyebrows — where powerful muscles draw the eyebrows inwards and downwards. These movements often express intense emotions : concentration, annoyance and stress.
Over time, repeated contractions leave their mark in the dermis, forming a lasting crease visible even at rest. This explains why creams or massages, whose action remains superficial, have only a limited effect : the cause lies deeper, in muscular dynamics.
Botulinum toxin directly addresses this mechanism. By temporarily inhibiting acetylcholine release at neuromuscular junctions, it blocks the action of the responsible muscles. This targeted relaxation allows the skin to relax progressively, clearly softening the wrinkle while preserving natural expression.
Results : what can you expect after botulinum toxin injections ?
Botulinum toxin is considered highly effective in the glabella when injected precisely. Patients report visible improvement within 3 to 7 days after injections, with the maximum effect reached around two weeks.
Clinically, the effects include :
- Disappearance or marked reduction of the vertical glabellar line or lines.
- Noticeable relaxation of the upper face.
- Eyes that appear more open, softer and less stern.
- A more neutral facial expression, often perceived as more rested.
In most cases, the dynamic wrinkle disappears completely, while the static wrinkle fades partially or even completely if still relatively mild. Results are generally reproducible and can be maintained with sessions spaced 4 to 6 months apart.
Scientific evidence on the effectiveness of botulinum toxin for frown lines
The effectiveness of botulinum toxin for frown lines has been demonstrated in numerous randomised controlled clinical studies. One of the most frequently cited, published in the Journal of the American Academy of Dermatology, shows that more than 85 % of patients receiving glabellar botulinum toxin injections notice a significant improvement in wrinkle severity, with high satisfaction even six months after treatment.
Multicentre trials conducted for European and American marketing authorisation have also validated botulinum toxin for this specific indication against strict criteria : clinical reduction of dynamic wrinkles, short- and medium-term safety, and reproducibility of results.
The tools used in these studies (such as the Glabellar Line Severity Scale, a validated visual scale) confirm significant wrinkle reduction within the first two weeks after injections, with a measurable effect in 90 % of cases.
Lasting results : how effective is treatment over time ?
Botulinum toxin has a temporary but prolonged action. Its effects last an average of 4 to 6 months, depending on initial muscle strength, frequency of facial expressions and individual characteristics. Over successive sessions, lasting improvement in the wrinkle is often observed even as the pharmacological effect fades, thanks to progressively reduced muscle activity in the treated area.
For some patients, particularly younger people or those with moderately active muscles, a single injection is enough to smooth the glabella for several months without a touch-up. Conversely, more expressive patients or those with already deep wrinkles benefit from a touch-up at 15 days, followed by maintenance sessions every six months.
It is important to remember that botulinum toxin is not permanent : its effectiveness is not curative. Nevertheless, regular use can slow the worsening of dynamic wrinkles and, in some cases, prevent their appearance.
Deep frown lines : is botulinum toxin still effective ?
When a frown line becomes visible at rest, it reflects changes in the dermis rather than expression alone. Botulinum toxin remains the reference treatment because it acts directly on the underlying mechanism : repeated muscle contraction. It therefore stabilises the wrinkle and relaxes the affected area.
When the skin crease is pronounced, hyaluronic acid filling can be combined with treatment. Their combined action optimises results : botulinum toxin targets the functional cause, while hyaluronic acid corrects the structural consequence.
Even when a frown line appears permanently established, botulinum toxin therefore remains the first-line treatment. Its targeted, reversible and well-tolerated action makes it central to managing expression lines.
A natural result or a frozen face ?
One of the main advantages of botulinum toxin in the glabella (between the eyebrows) is that it achieves a visible result without freezing the face. Appropriately dosed and precisely injected, it relaxes the area without blocking natural expressions. The eyes remain lively, the eyebrows retain movement and facial expressiveness is respected.
Studies show that side effects are very rare, particularly when treatment is performed by an experienced doctor. Overly frozen or artificial results are exceptional in this setting. Botulinum toxin's effectiveness depends on precise knowledge of facial anatomy and respect for muscular dynamics rather than a large quantity of product.
