HOW LONG DO BOTULINUM TOXIN INJECTIONS FOR FROWN LINES LAST ?

Last updated : 12 June 2026

Frown lines, or glabellar lines, are among the first expression lines to develop. Resulting from repeated contraction of the corrugator and procerus muscles — particularly during frowning — they give the eyes an expression sometimes perceived as stern, tense or worried.
The reference treatment for smoothing these wrinkles is botulinum toxin injections. This medicine temporarily relaxes the responsible muscles.
One of the topics patients most frequently discuss during consultations is treatment longevity : how long will botulinum toxin's effects persist, and when should another session be considered ?
Botulinum toxin effects last between 4 and 6 months.

How botulinum toxin works on frown lines

Botulinum toxin blocks the release of acetylcholine, a neurotransmitter essential for voluntary muscle contraction. Injected at very low doses into the glabellar muscles, it selectively reduces muscle activity, allowing the overlying skin to relax and the wrinkle to gradually soften.
The effect is not immediate : initial relaxation generally appears after three to five days, with maximum effectiveness between the 10th and 14th day. The muscle then remains partially or completely inactive for several weeks until nerve endings regenerate. This neuromuscular reconnection determines how long the therapeutic effect lasts.

How long do botulinum toxin effects on frown lines last ?

For most patients, the visible effects of botulinum toxin injections on frown lines last between 4 and 6 months. However, substantial individual variation depends on several factors :

  • Initial muscle strength : very powerful glabellar muscles, common in men or younger patients, may reduce result longevity.
  • Injected dose : an insufficient amount reduces lasting effectiveness, while an appropriate dose clearly prolongs the effect.
  • Patient metabolism : some people, particularly very athletic patients, metabolise the toxin faster, potentially shortening its action.
  • Treatment consistency : regular maintenance sessions allow progressive muscle relaxation through functional deafferentation, encouraging longer-lasting effectiveness over time.

Under optimal conditions, results last an average of five months, sometimes with residual effects up to six or seven months, particularly in patients receiving regular repeat injections.

How do frown lines change between injection sessions ?

Over the weeks after injections, the toxin's effect gradually fades. Muscle activity returns through gradual recovery of movement rather than abruptly. In practice :

  • Between 1 and 2 months : effects are maximal, with complete muscle blockade and an inability to frown.
  • Between 3 and 4 months : the muscle begins recovering partial activity.
  • Between 5 and 6 months : muscle contraction becomes visible again and frown lines gradually reappear.

In many cases, however, the wrinkle never regains its original depth thanks to muscle weakening acquired over successive sessions.

Can botulinum toxin's effects be prolonged ?

Although biological factors largely determine how long the toxin acts, certain strategies can optimise its longevity :

  • Maintain regular injection intervals : treatment every 4 to 5 months contributes to progressive muscle weakening, reducing contraction strength.
  • Avoid deliberate contractions after injections : during the following days, it is preferable not to overwork the forehead and glabella, which could encourage toxin diffusion and compromise the result.
  • Adopt suitable skincare : regular hydration, effective sun protection and targeted anti-ageing products (antioxidants, peptides, retinoids) help prolong benefits.
  • Combine other techniques : Skinboosters or radiofrequency, for example, improve skin quality and enhance the overall aesthetic effect.

However, injecting a larger dose does not prolong treatment. Excess dosing only increases adverse-effect risk without additional benefit for result longevity.

What happens when the effects fade ?

As botulinum toxin wears off, nerve endings gradually resume their function and muscles regain their initial tone. In patients who do not repeat treatment, frown lines eventually return over a period that varies according to initial skin condition.
Botulinum toxin never worsens a wrinkle. On the contrary, it rests the area for several months, slowing progression. Patients who stop treatment regain their natural expression without rebound deterioration.

Does “muscle memory” eventually allow longer intervals between sessions ?

Several clinical studies have shown that a muscle kept at rest for several consecutive years loses tone. It partially atrophies, leading to weaker contraction and therefore a less pronounced wrinkle, even outside active treatment periods.
After 3 or 4 years of regular treatment, some patients can therefore extend sessions to once every 7 to 8 months while maintaining a good aesthetic result. This phenomenon, called muscular deconditioning, is particularly observed in glabellar treatment, where reflex contractions are very frequent.

Does treatment last equally long in every patient ?

Treatment longevity differs between patients, with substantial individual variation influenced by several factors :

  • Sex : men often have stronger muscles, so effects last less long.
  • Age : younger patients generally have less intense muscle activity, which may prolong relaxation.
  • Phototype and skin quality : thin, lax or sun-damaged skin tends to crease more, even while the muscular effect remains.
  • Treatment history : patients with several previous sessions often respond better and for longer thanks to a cumulative effect.

It is therefore essential to tailor the protocol to each patient rather than rely on a theoretical average.

Frown lines : an area particularly responsive to botulinum toxin

The glabella, between the eyebrows, is a highly active muscular area. It contracts frequently, often unconsciously, whether at rest, under stress, during concentration or even while reading. This almost constant activity explains early, pronounced wrinkles in this region.
This also explains why botulinum toxin is particularly effective here. However, muscular overactivity may cause its effect to last slightly less long than elsewhere on the face. Glabellar muscle activity is therefore highly reversible, requiring a rigorous maintenance protocol to preserve lasting smoothing.

Does longevity differ according to the type of botulinum toxin used ?

Although botulinum toxin brands share a common mechanism, they differ in molecular structure, protein weight, tissue diffusion and stability. These differences may marginally influence longevity.
For example, some studies suggest that certain toxins, through their protein composition and more targeted diffusion, may last slightly longer than alternatives. One particular brand is often preferred for purity but may act for slightly less time in some patients. These variations remain moderate, but an experienced doctor can select the toxin best suited to the patient's muscular profile, aesthetic objective and desired session frequency.

Why are botulinum toxin's effects on frown lines not permanent ?

Botulinum toxin temporarily inhibits neuromuscular transmission. After injection, it prevents acetylcholine release, blocking the contraction signal to target muscles. However, this inhibition does not irreversibly damage nerves or muscle fibres.
The peripheral nervous system can regenerate : over the weeks, new synaptic junctions form, gradually restoring motor function. This neuromuscular recovery explains why botulinum toxin's effects are temporary. Repeat injections provide controlled treatment with opportunities to adjust doses and strategy at each session.

What signs indicate that botulinum toxin's effects on frown lines are wearing off ?

Botulinum toxin's effects disappear gradually, never suddenly. The first sign is often a slight return of movement in the glabella : patients notice a small frown when expressing emotion or concentrating. This sometimes goes unnoticed because the wrinkle remains subtle at rest.
Over time, muscle contractions intensify and the skin starts creasing again. Without further injections, the frown line eventually becomes visibly apparent. It may return to its initial appearance or look slightly softer, depending on skin quality and the consistency of previous treatments.

How long do hyaluronic acid injection results for frown lines last ?

Results from hyaluronic acid injections into frown lines generally last between 10 and 18 months, depending on several intrinsic and extrinsic factors. The hyaluronic acid used is often lightly cross-linked, meaning stabilised to resist the body's natural enzymatic breakdown. However, this duration is indicative : it depends on movement in the treated area, the acid type, wrinkle depth and patient metabolism. Because the glabella is frequently used in facial expressions — particularly frowning — injected product experiences constant mechanical stress, potentially accelerating absorption. Effects may last longer in younger patients or those treated preventively, while patients with pronounced static wrinkles are often advised to have maintenance every 9 to 10 months for optimal results. Finally, combining hyaluronic acid with prior botulinum toxin injections can substantially prolong filling because the gel undergoes less daily muscular tension.
