HOLLOW EYE, EMPTY UPPER EYELID : FILLING BY INJECTION OF HYALURONIC ACID

What is a hollow eye?

The hollow eye is an empty eye at the level of the upper eyelid. It is to be distinguished from a round eye where the sclera (white part of the eyeball) is less covered by the eyelids. A hollow eye often gives the impression of a round eye, but these are two different conditions. Hollow eyes age the eyes and make them look tired. A young eye is a full eye!

Why does the eye become hollow?

The hollowing of the eye is the result of a loss of fat and/or bone volume.

The hollowing of the upper eyelids can be caused by a number of factors.

  • Ageing: over time, the bone resorbs and the skin loses its elasticity, causing the tissues to sag.
  • Loss of collagen: the reduction in collagen and elastin production makes the skin thinner and more fragile.
  • Aggressive blepharoplasty: removal of too much fat.
  • Genetics: some people are naturally predisposed to hollow eyes.
  • Lifestyle: stress, lack of sleep and an unbalanced diet can exacerbate the problem.

How to stop sunken eyes?

To reduce the appearance of sunken eyes, you need to identify the factors responsible.
Often, this condition is due to a loss of volume which causes a hollow and tired appearance.
To remedy this, the non-surgical option available in aesthetic medicine is the injection of hyaluronic acid.
Hyaluronic acid injections are particularly effective because they restore lost volume and fill hollows naturally and harmoniously. This safe and meticulous procedure is used to restructure the orbital area and reduce the appearance of sunken eyes. The face looks more rested and rejuvenated.

How can hollow eyes be prevented or reduced?

There is no real way to prevent the appearance of sunken eyes. Skin care and lifestyle influence the hydration and luminosity of the skin in this area.
It is therefore advisable to :

  • Maintaining good hydration is essential for healthy skin.
  • Skin care: moisturising creams and serums containing ingredients such as hyaluronic acid, vitamin C and retinol help to maintain skin elasticity.
  • Apply sun protection to prevent premature ageing of the skin.
  • Eat a balanced diet.
  • Get enough quality sleep.
  • A healthy lifestyle.

How to fill hollow eyes?

The upper eyelid is filled by injection of hyaluronic acid in aesthetic medicine and by lipofilling of the upper eyelid in cosmetic surgery.

Dr Romano uses a non-hydrophilic hyaluronic acid, such as Restylane, which is injected with a cannula along the upper orbital rim.
The number of syringes used varies between one and three, beyond which the doctor recommends surgical lipofilling.
Filling must be gradual to allow the product to settle correctly.
Dr Romano often injects a single syringe (for both eyes) per session. The sessions should be spaced two months apart.
Given the technical difficulties involved, this injection is only carried out by a small number of practitioners.

Hyaluronic acid for filling hollow eyes

The injection ofhyaluronic acid to correct sunken eyes is a quick and easy procedure.
During the initial consultation, Dr Romano analyses the patient's anatomy, discusses his or her expectations and explains the possible outcome.

In fact, the best and sometimes only solution for excessively large hollows is surgery.
Significant sagging of the skin on the upper eyelid may be more obvious after the injection. In these cases, Dr Romano will inform you and, in most cases, advise against the injection.

Hyaluronic acid is carefully injected into the upper orbital rim, against the bone and not into the upper eyelid, using a cannula. The cannula is a needle with a round tip.
Injection into the upper eyelid causes permanent swelling, ball formation and the Tyndall effect

The procedure generally takes about ten minutes and is not very painful.
After the procedure, patients are asked to avoid sun exposure and physical activity for the first 24 hours.

Safety of hyaluronic acid injections in the eyes

The safety of hyaluronic acid injections around the eyes is a major concern for patients and doctors alike.
This procedure, although minimally invasive, must be carried out by an experienced and qualified doctor to ensure optimal results and to avoid any risks.

In terms of results, a botched injection can cause :

  • Surface irregularities and a wave-like appearance.
  • Closure of the manhole due to overfilling.
  • Permanent oedema.
  • The Tyndall effect: the acid turns purple.
  • Apparent balls due to injection in too shallow a plane.

For a safe injection, you need to consult an experienced practitioner who has :

  • Perfect knowledge of the anatomy of the orbital region.
  • Complete technical control of the procedure.
  • A fairly sophisticated ability to analyse the orbital zone.

Results before/after hollow eye treatment

The result is not really immediate. The eyelids are swollen at the end of the session. This swelling increases to a greater or lesser extent in the days that follow.
The final result is obtained three weeks after the injection. The eyes look fresher and younger. The face appears more luminous.
Haematomas are rare.
Re-injections are carried out every two years, and only one syringe is needed, regardless of the quantity used for the first injection.
