They often create a tired, sad or aged appearance, sometimes completely at odds with the patient's health or inner feelings. Yet \u201cdark circles\u201d encompasses very different anatomical realities, explaining why effective treatments vary considerably between people. Should dark circles be treated with aesthetic medicine or should cosmetic surgery be considered? There is no universal answer. They are frequently associated with thin skin, visible blood vessels and genetic or pigmentation factors.<\/li>\n<li>Under-eye bags: a prominence caused by protrusion of lower-eyelid fat compartments, sometimes associated with excess skin.<\/li>\n<li>Mixed forms: in many cases, several mechanisms coexist, for example hollows, bags and discolouration.<\/li>\n<\/ul>\n<p>Treatment choice depends directly on this diagnostic assessment. A hollow is not corrected in the same way as a bag, and excess skin is not treated like simple discolouration. Precise assessment is therefore essential for suitable, coherent care.<\/p>\n<h2>Aesthetic medicine: when is it enough?<\/h2>\n<p>Aesthetic medical procedures most often provide an appropriate initial approach, particularly when one of the following predominates:<\/p>\n<ul>\n<li>Le creux est le probl\u00e8me principal : les injections d\u2019acide hyaluronique, r\u00e9alis\u00e9es par des m\u00e9decins exp\u00e9riment\u00e9s, comblent le creux du cerne et \u00e9ventuellement celui de la vall\u00e9e des larmes. Elles ne conviennent toutefois pas \u00e0 tous les profils \u2014 notamment en cas de peau tr\u00e8s fine ou de poches importantes \u2014 mais, lorsque l\u2019indication est bien pos\u00e9e, elles adoucissent l\u2019expression fatigu\u00e9e sans modifier l&rsquo;expressivit\u00e9 du regard.<\/li>\n<li>Dehydrated, thin or creased skin is the main problem: mesotherapy and certain gentle peels or non-aggressive lasers can improve lower-eyelid skin quality, radiance and quality, helping reduce a tired appearance.<\/li>\n<li>Discolouration predominates: depending on its origin, targeted topical care, specific peels or lasers for certain indications may be offered. However, it should be noted that dark pigmented circles are sometimes difficult to remove completely.<\/li>\n<\/ul>\n<p>These approaches have the advantage of being gradual, adjustable and reversible. However, they require excellent knowledge of orbital anatomy and rigorous patient selection, particularly for hyaluronic acid injections, as this area is delicate and at risk of overcorrection.<\/p>\n<h2>Cosmetic surgery: when should it be preferred?<\/h2>\n<p>Cosmetic surgery, particularly lower blepharoplasty, is indicated when the concern is mainly associated with:<\/p>\n<ul>\n<li>clearly visible fat bags, present even at rest, which do not diminish with rest or local care;<\/li>\n<li>excess lower-eyelid skin causing very crepey skin and deep wrinkles, when a medium-depth peel cannot be performed or would be insufficient to address the aesthetic concern;<\/li>\n<li>marked lower-eyelid laxity that can no longer be corrected with injections or superficial treatments.<\/li>\n<\/ul>\n<p>Depending on the indication, blepharoplasty removes or repositions excess fat responsible for bags; moderately tightens skin and redefines the eyelid contour; and, in some cases, combines an additional procedure on the muscle or outer canthus to improve eyelid stability.<br \/>This is a more invasive procedure performed in an operating theatre, with longer postoperative recovery but lasting results. It is indicated for genuine bags or excess skin rather than mild dark circles.<\/p>\n<h2>Medicine or surgery: how should the choice be guided?<\/h2>\n<p>Care is always determined individually following a personalised consultation.<br \/>When hollowing predominates in a young or middle-aged patient without notable bags, aesthetic medicine is usually the first-line option. It particularly relies on hyaluronic acid injections specifically adapted to this delicate area, combined with treatments to improve skin quality.<br \/>If the concern combines moderate hollowing, a slight change in skin tone and mild discolouration, a comprehensive treatment strategy may be considered.
