Wounds close more slowly, scars form later and skin repair becomes less effective. Long observed clinically, this phenomenon has recently been confirmed and analysed molecularly by several research teams, including Rockefeller University. Their work precisely explains the biological mechanisms behind slower healing with age, revealing complex cellular alterations and disrupted communication between tissues.
Skin ageing involves more than wrinkles or loss of tone. It is a comprehensive biological process involving gradual decline in essential skin functions, particularly protection, regeneration and repair. Healing is one of the human body's most sophisticated mechanisms: it simultaneously mobilises immune cells, keratinocytes, fibroblasts and biochemical signals to rebuild uniform, functional tissue. With age, each link becomes disrupted, slowing the entire process.

Book an appointment

    Contents

The normal healing process

In a healthy young person, healing occurs through coordinated stages. Immediately after injury, an inflammatory phase begins: blood vessels contract, platelets aggregate and a clot forms to limit bleeding. This rapid response also releases chemical mediators attracting immune cells to the wound.
Next comes proliferation, when fibroblasts produce collagen and keratinocytes migrate from wound edges to form new epidermis. Finally, remodelling or maturation strengthens newly formed tissue and gradually restores elasticity and resilience.
Every stage requires perfect coordination between skin cells, immune cells and the molecular messengers regulating their activity. This constant intercellular dialogue determines healing speed and quality.

Profound changes with ageing

Through an animal-model study, Rockefeller University researchers showed that ageing profoundly alters these cellular interactions. In young mice aged two months, equivalent to twenty human years, skin regeneration is rapid: five days after injury, keratinocytes have already migrated to fill the wound bed. In older mice, however, the process is markedly slower: keratinocytes take much longer to move and cover the injured area, delaying overall healing.
Researchers identified several causes of this slowdown. One is weakened communication between keratinocytes and immune cells. Normally, these cells exchange signals through specific proteins called Skint, coordinating repair. With age, production of these proteins decreases, disrupting tissue dialogue and slowing new skin formation.
This signalling deficit accompanies a general decline in cellular activity: keratinocytes divide more slowly, fibroblasts produce less collagen and immune responses become less effective. This systemic weakening of communication resembles an orchestra whose instruments no longer play at the same tempo.

The immune system's role in healing

Skin and the immune system are closely linked. When a wound occurs, the body triggers controlled inflammation to remove cellular debris and prevent infection. Well regulated, this process subsequently promotes regeneration. With age, however, the response becomes less precise.
In older people, innate immunity is less responsive. Macrophages and neutrophils move more slowly to wounds, release fewer growth factors and coordinate cell proliferation less effectively. Furthermore, "inflammaging", chronic low-grade inflammation typical of ageing, disrupts the balance between tissue destruction and reconstruction. This diffuse but mild inflammation sustains constant oxidative stress, impairing tissue quality and slowing regeneration.
Ageing skin therefore lacks not only repair cells but also a biological environment favourable to their action.

Declining skin regenerative capacity

With age, skin thins, loses elasticity and its vascular network diminishes. These structural changes directly affect healing. Fibroblasts, responsible for collagen and elastin production, become less active. The dermis loses extracellular matrix, and blood supply to injured areas takes longer to recover.
Keratinocytes also show reduced migration and proliferation. Their renewal cycle lengthens, and the skin barrier takes longer to reform. Meanwhile, epidermal stem cells, essential for initiating repair, become rarer and less effective.
These changes explain why a small wound healing within days in a young person may require several weeks in an older person. This slowdown is not solely aesthetic: it also increases infection, hypertrophic scarring and chronic ulceration risks.

Aggravating environmental factors

Skin ageing does not depend solely on time. External factors also influence healing quality. Repeated sun exposure, pollution, smoking or unbalanced nutrition accelerate oxidative stress, reduce microcirculation and impair collagen production.
Lifestyle therefore plays a decisive role. Well-hydrated skin protected from ultraviolet rays and nourished by an antioxidant-rich diet maintains better repair capacity. Conversely, skin weakened by sun or dehydration heals less easily.
Hormonal status also matters: falling oestrogen after menopause reduces vascularisation and collagen synthesis, both essential for regeneration. In men, gradually declining testosterone may also slow healing.

Understanding for better prevention

Better understanding of skin ageing mechanisms allows medical care to be adapted. Current research explores several therapeutic avenues to stimulate cellular regeneration and restore communication between different skin cells.
Some approaches, such as PRP (Platelet-Rich Plasma) injections, use the regenerative properties of growth factors in the patient's plasma. By promoting keratinocyte migration and collagen production, PRP can improve healing and skin tissue quality. Other treatments, including fractional lasers and collagen stimulators, stimulate dermal synthesis and encourage skin to restart natural repair mechanisms.
These aesthetic medicine techniques perfectly illustrate the convergence of medical care and skin health maintenance. They help restore skin physiology and functionality.

The future of wound healing research

Advances in cell biology open promising prospects. Scientists now examine signalling pathways altered by age and seek to reactivate them. One aim is to restore dialogue between keratinocytes and immune cells, particularly by modulating proteins such as Skint.
Other research concerns cellular reprogramming and gene therapy to prolong tissue regenerative capacity. Detailed understanding of skin ageing could enable treatments that accelerate healing and slow overall skin ageing.

Skin ageing: a physiological phenomenon, not an inevitability

Slower healing with age naturally results from weakened biological functions, but is not inevitable. A preventative approach and appropriate stimulating care can preserve skin vitality.
Aesthetic medicine fully embraces this approach. Through regenerative techniques and non-invasive methods, it maintains tissue health, stimulates microcirculation and strengthens skin repair capacity. Within a rigorous medical framework, these treatments prolong skin's functional youthfulness beyond appearance.

Conclusion: skin that repairs more slowly but can still regenerate

With age, skin heals more slowly and less effectively but retains regenerative potential that can be stimulated. Recent discoveries show that this slowdown primarily reflects weakened cellular communication and altered biological repair signals.
Understanding these mechanisms better supports skin ageing by combining prevention, dermatological care and appropriate aesthetic medical procedures. Even an ageing body can repair itself: it simply needs more time and a favourable environment.

Article written by Dr Romano Valeria

LinkedIn

dr.romano.valeria@gmail.com

+41 78 209 02 89

Cicatrisation ralentie avec l’âge : explication des mécanismes biologiques impliquant diminution de la synthèse du collagène, altération cellulaire et circulation réduite, ainsi que l’intérêt de ces données pour la prise en charge esthétique à Genève

[1

post-45328","post","type-post","status-publish","format-standard","has-post-thumbnail","hentry","category-uncategorized
<!-- This site is optimized with the Yoast SEO plugin v28.7 -
