Depending on the indication, this may be melanin, particularly for pigmentation lesions or hair removal ; haemoglobin, when treating redness and vascular lesions ; or water, the main target of fractional lasers used for resurfacing and skin remodelling.
The absorbed light energy is converted into controlled heat, allowing either targeted destruction of unwanted structures or thermal stimulation of the dermis.
This thermal interaction underlies the clinical effectiveness of lasers while also explaining the possibility of side effects associated with the tissue response induced.

Common immediate side effects of laser treatment

After treatment, temporary reactions may develop in the treated area, such as local sensitivity, slight redness or swelling. These are generally mild and resolve spontaneously within a few days.
The most common immediate side effects are expected and reflect the physiological inflammatory response induced by laser treatment. In most cases, they are temporary and moderate in intensity.
Redness after laser treatment is almost universal following fractional or vascular laser procedures. It results from local vasodilation and activation of the inflammatory mediators needed for tissue repair. Its duration varies from a few hours to several days according to treatment intensity. After an ablative laser, redness may persist longer, sometimes for up to two to three weeks.
Moderate swelling may also develop, particularly around the eyelids or cheeks, where subcutaneous tissue is looser. It generally peaks between 24 and 48 hours after the session before resolving spontaneously. It is more common after ablative treatment or the use of higher settings.
A sensation of warmth or moderate burning, comparable to sunburn, may occur in the hours following treatment. This discomfort usually resolves within 24 to 72 hours.

Peeling and crusting : normal after-effects of certain lasers

After fractional ablative or pigment laser treatment, peeling, fine crusting and temporary skin dryness are common.
These changes reflect the physiological process of epidermal renewal and removal of tissue altered by thermal energy. They indicate activation of skin repair mechanisms and form part of the normal course of healing.
They are not complications but expected stages in the regenerative process induced by treatment.

Pigmentation marks after laser treatment

Pigmentation changes are among the side effects most closely monitored after laser treatment.
Post-inflammatory hyperpigmentation is a temporary darkening of the treated area. It results from excessive stimulation of melanocytes (the cells that produce melanin) following laser-induced inflammation. This is more frequently observed in patients with darker phototypes, after early sun exposure following the session or when excessively aggressive settings are used. In most cases, hyperpigmentation resolves spontaneously over several weeks to a few months.
Hypopigmentation is rarer. It involves a partial reduction in skin pigmentation and may follow deep ablative treatments, unsuitable settings or abnormal healing. When treatment is performed by an experienced doctor with appropriate patient selection and settings, this complication is exceptional.

Risk of skin burns with lasers

Laser treatment converts light energy into controlled heat. When settings are unsuitable, this rise in temperature can cause a skin burn, most often superficial and, exceptionally, deeper.
The main factors contributing to this complication are inadequate assessment of skin phototype, inappropriate device calibration or insufficient skin cooling.

Scarring after laser treatment

Scarring after laser treatment is rare, estimated at less than 1 % in contemporary dermatological studies, but remains theoretically possible.
It may occur following a secondary infection of the treated area, in patients predisposed to abnormal scarring, such as those with a history of keloids, or after excessively aggressive treatment.
A thorough, rigorous medical history before the session identifies individual risk factors and allows the treatment strategy to be adapted to minimise this possibility.

Infection after medical laser treatment

After medical laser treatment, the skin barrier is temporarily impaired, which may favour infection, although this complication is rare. This may involve a local bacterial infection or reactivation of herpes.
The risk is higher in patients with a history of cold sores and when post-treatment care recommendations are not followed.
In patients identified as being at risk, antiviral prophylaxis may be prescribed to prevent herpes reactivation and support a safe healing period.

Side effects specific to each type of laser

Side effects vary according to the laser technology used, with each device having a specific tolerability profile.
Fractional ablative laser treatment is associated with more prolonged redness, pronounced peeling and increased skin sensitivity during healing. There is a moderate risk of pigmentation changes, particularly in darker phototypes or following inappropriate sun exposure.
Non-ablative fractional laser treatment generally causes mild redness and moderate swelling, with a low risk of pigmentation changes when settings are adapted to the phototype.
Vascular lasers may cause temporary bruising, transient redness and, more rarely, purpura associated with fragility of the treated capillaries.
Hair removal lasers frequently cause follicular redness and swelling around the hair follicles, reflecting the expected inflammatory response. Burns are rare but may occur on recently tanned skin or when sun exposure precautions are not adequately followed.
Each technology therefore requires a precise indication, personalised assessment and appropriate settings to optimise tolerability and treatment safety.

Factors influencing side effects after laser treatment

Side effects after laser treatment depend on several factors related to the patient and the conditions in which the procedure is performed.
Skin phototype is central to the choice of settings, as is recent sun exposure, which may increase the risk of pigmentation changes. The doctor must also check for photosensitising medicines and active skin conditions that may impair healing.
The doctor's expertise, the quality of the initial diagnosis and mastery of technical settings play a central role in treatment safety. Strict adherence to post-procedure care also determines a favourable recovery.
A thorough medical assessment, combined with clear patient information, significantly reduces the likelihood of complications.

Does laser treatment have long-term side effects ?

The question of long-term effects frequently causes concern. Current scientific evidence indicates that medical lasers, when used for appropriate indications in a rigorous medical setting, do not cause accelerated skin ageing.
Nor do they cause lasting skin fragility. On the contrary, their action relies on controlled stimulation of tissue repair mechanisms and induction of neocollagenesis, contributing to a gradual improvement in dermal quality.
Late complications remain rare. When they occur, they are most often associated with an inappropriate indication, excessive settings or treatment performed outside a properly controlled medical setting.

How can side effects of medical lasers be prevented ?

Preventing side effects from medical lasers requires a structured, rigorous approach.
It begins with a thorough consultation including analysis of the skin phototype, assessment of medical and dermatological history and a detailed skin examination. This confirms the indication, identifies risk factors and allows the treatment strategy to be adapted.
Device settings are a determining factor. Wavelength selection, precise adjustment of fluence and adaptation of settings to skin type determine both effectiveness and safety.
Sun protection is essential before and after the session. Sun exposure should be avoided, and regular application of a broad-spectrum sunscreen is recommended to limit the risk of pigmentation changes.
Finally, following post-laser care instructions supports optimal healing. Appropriate hydration, reparative skincare and avoidance of irritating products help skin regeneration and reduce the risk of complications.

Are medical lasers safe ?

Medical lasers are considered safe devices when used in a rigorous medical setting, for an appropriate indication and with technology that meets current standards.
Contemporary clinical studies report a low rate of severe complications.
Most observed side effects are mild, temporary, predictable and reversible. They most often reflect the physiological inflammatory response induced by treatment and form part of normal skin repair.

Article written by Dr Romano Valeria

LinkedIn

dr.romano.valeria@gmail.com

+41 78 209 02 89

["post-61413","infographie","type-infographie","status-publish","hentry
