A side effect is an expected or predictable response of the body to a product or medical procedure : it is generally mild, temporary, and familiar to the doctor. For example, slight swelling after an injection is a typical side effect.
A complication, by contrast, occurs unexpectedly, abnormally or excessively. It may result from an inappropriate indication, a technical error, or an unpredictable individual reaction. Managing a complication often requires close medical follow-up or even specific treatment.
Understanding this distinction helps patients understand the normal after-effects of an injection and avoid confusing an expected reaction with a genuine medical problem.

Common, mild and temporary side effects of injections

Most side effects observed after an aesthetic injection are part of the body’s normal response to the skin being punctured and do not constitute complications.

Haematomas and bruising : these are the most common side effects. The needle or cannula can damage a small capillary and cause local bruising. This disappears spontaneously within a few days, without treatment. Simple precautions, such as avoiding aspirin, anti-inflammatory medicines or certain dietary supplements before the injection, can reduce this risk.

Redness, oedema, local tenderness : slight oedema (swelling) or redness at the injection site is normal, especially in areas where the skin is thin, such as the lips or under-eye area. These signs disappear within 24 to 72 hours, sometimes sooner. They can be reduced by applying cold locally and do not require specific treatment unless they persist.

A feeling of tightness or a foreign body : some patients describe an unusual sensation in the injected area, particularly after volumising filler treatment. This sensation is temporary and generally disappears within one to two weeks, as the product integrates smoothly into the tissue.

Moderate or delayed side effects

Asymmetry or overcorrection : asymmetry can result from uneven injection or a different tissue response on one side of the face compared with the other. Overcorrection occurs when the amount of product exceeds what is aesthetically necessary. These imbalances can generally be corrected easily with a small adjustment during a touch-up.

Subcutaneous nodules : small areas of firmness can sometimes form beneath the skin, particularly in mobile areas or where the skin is thin. These correspond to localised product accumulations, often poorly distributed or injected too superficially. These nodules are usually painless and temporary but must be monitored. If they persist, targeted massage or dissolution may be considered.

Delayed inflammatory reactions : more rarely, patients may develop a localised or widespread inflammatory reaction several weeks or even months after the injection. These reactions are usually mild but can look dramatic : swelling, redness, discomfort. They can be triggered by an intercurrent viral infection (flu, COVID), immune stress (vaccination, autoimmune reaction), or a poorly tolerated product. In these cases, appropriate medical care, sometimes with anti-inflammatory treatment or a local corticosteroid, is sufficient.

Rare but serious complications

Vascular embolism : the most feared complication – although rare – is vascular embolism. It occurs when the injected product accidentally enters a blood vessel, blocking local circulation. This can cause skin necrosis or even irreversible blindness if an artery around the eye is affected. These exceptional cases justify injections being performed exclusively by doctors trained in facial vascular anatomy and able to act immediately : hyaluronidase injection, vascular massage, local anticoagulation.

Infection : any break in the skin carries a risk of infection, however small. If strict medical asepsis rules are followed, this risk is almost zero. Conversely, injections performed outside a medical setting in questionable hygiene conditions expose patients to cellulitis, abscesses or delayed bacterial infections that can sometimes be serious.

Side effects specific to botulinum toxin

Although different from fillers, botulinum toxin can also cause certain side effects, generally mild and temporary :

Ptosis (drooping eyelid) : caused by unintended diffusion of the product into an adjacent muscle. The effect is temporary but may last a few weeks.

An asymmetrical smile or drooping eyebrow : linked to poor distribution of the toxin.

Headaches or forehead tightness in the days following the injection, often linked to muscular adaptation.

Tous ces effets disparaissent avec l’élimination du produit, sans séquelle.

Product traceability : an essential safeguard for preventing adverse effects

An often overlooked factor in preventing side effects is the quality and traceability of the injected products. Improperly stored hyaluronic acid, an unauthorised or counterfeit product, or incorrectly dosed botulinum toxin can cause unexpected reactions that are more frequent and harder to treat.
This is why every conscientious doctor uses only CE-marked or Swissmedic-approved products, maintains complete traceability (batch, manufacturer, injection date), and gives the patient a traceability label for the product used.

Who is most at risk of side effects ?

Certain patient profiles are more likely to experience adverse effects :

People with autoimmune diseases or chronic inflammatory conditions.

People who have previously had allergic reactions to an injectable product.

Patients injected by non-doctors.

People who have received multiple injections of different products within a short period, without an overall strategy.

These situations warrant rigorous medical assessment and greater caution when determining indications.

Side effects according to the injected area

Not all facial areas carry the same risk of side effects. Some have a richer blood supply, are more sensitive or move more, which can influence the nature and frequency of the reactions observed.

The lips : prone to post-injection swelling, which can sometimes be dramatic, because of their rich blood supply and constant movement.

The under-eye area : a delicate area with a limited blood supply and thin skin. Poorly selected or incorrectly injected hyaluronic acid can cause a Tyndall effect (a bluish reflection beneath the skin), visible irregularities or persistent water retention.

The nose : an area with a high vascular risk. Any injection in this region must be performed with extreme caution to avoid a serious complication (necrosis or embolism).

Nasolabial folds and marionette lines : areas prone to small surface irregularities in cases of overcorrection or overly superficial injection. There is also a risk of vascular occlusion and therefore necrosis.

Adapting the product choice, injection technique and depth to the area treated is fundamental to limiting adverse effects.

The role of injection technique in the occurrence of side effects

The occurrence of side effects depends strongly on the technique used by the doctor. Several factors influence the risk :

Type of instrument : needle or cannula. A cannula is often preferred to limit bruising and avoid injecting into a vessel. Its use is compulsory in areas with a vascular risk.

Injection depth : an overly superficial injection can create visible irregularities, whereas an excessively deep injection may be ineffective, meaning the result will not be visible.

Injection speed : in inexperienced hands, an injection performed too quickly (relative to the speed at which the product is introduced) can cause excessive swelling or inflammation or poor product distribution.

L’expérience du médecin et sa connaissance de l’anatomie sont donc déterminantes pour éviter les effets secondaires techniques, même lorsque le produit est parfaitement toléré par le patient.

Can you develop an intolerance or allergy to injected products ?

Allergic reactions to modern fillers are extremely rare, particularly because the substances used – such as hyaluronic acid – are biodegradable, biocompatible and very similar to those naturally produced by the body.
However, in very rare cases, the following may occur :

A delayed immune reaction (localised chronic inflammation, persistent redness).

Hypersensitivity to certain additional components, such as cross-linking agents or preservatives.

These exceptional cases are managed by a competent doctor using anti-inflammatory or enzymatic treatments. It is therefore essential for the injection to be preceded by a comprehensive medical discussion, particularly in patients with immune conditions or a history of allergies.

Article written by Dr Romano Valeria

LinkedIn

dr.romano.valeria@gmail.com

+41 78 209 02 89

["post-28638","infographie","type-infographie","status-publish","hentry
