It relies on controlled application of intense cold capable of inducing fat-cell apoptosis without damaging surrounding tissues. Nevertheless, like any medical procedure, it carries potential side effects. Burns or skin damage are among the concerns patients frequently raise. Are these concerns justified ? This controlled cooling weakens adipocyte membranes and triggers programmed self-destruction while preserving surrounding tissues — skin, nerves and blood vessels. Effectiveness relies on highly precise temperature control. A temperature that is too high will not produce the intended cell destruction, while excessive or unevenly distributed cold can cause skin injuries resembling burns. Cryolipolysis must therefore always be performed with a certified medical device by a specifically trained doctor in a medical setting.

What types of burns or injuries may occur ?

Although rare, skin-related adverse effects have been documented in the literature. Three main types of reaction are distinguished :

Superficial freezing injury (mild frost injury) : this is the most frequent form. It causes persistent redness, localised swelling, burning or tingling and sometimes skin peeling in the following days. This reaction is temporary and reversible without lasting effects, provided sun exposure is avoided during the inflammatory phase.

Superficial second-degree burn : this appears as a blister, often accompanied by moderate pain and more pronounced redness. It may result from inadequate thermal insulation, incorrect applicator positioning or prolonged exposure. With proper care, it heals without marks but requires careful medical follow-up and appropriate dermatological care.

Necrotic injury : this exceptional adverse effect occurs when temperature or exposure duration exceeds skin tolerance thresholds. It appears as an open wound progressing towards superficial necrosis and requires specialist care. Healing is slower, and an atrophic or pigmented scar may persist without appropriate treatment.

How common are burns from cryolipolysis ?

Clinical studies show that burns or skin injuries after cryolipolysis are extremely rare when treatment is performed under appropriate conditions. Large published series estimate their frequency at less than 0,1 %, meaning fewer than one case per thousand sessions. This risk increases markedly when the procedure uses a non-medical device, often for home or unsupervised cosmetic use, or is performed by an untrained operator, as sometimes occurs in unregulated salons. It also increases without prior skin examination or consideration of dermatological history. The risk therefore lies in how cryolipolysis is performed rather than the technique itself. A rigorous protocol, reliable equipment and the doctor's competence remain the true foundations of treatment safety.

Which factors increase the risk of burns ?

Before considering cryolipolysis, certain situations must be recognised as risk factors :

Thin or fragile skin, particularly on the neck, arms or areas previously treated with lasers, peels or corticosteroids, may react unpredictably to cold.

Increased cold sensitivity or conditions such as Raynaud's syndrome, cryoglobulinaemia or cold urticaria are contraindications to the procedure.

A hernia, circulatory disorder or reduced local sensation requires thorough medical assessment before any session.

A history of slow healing or post-inflammatory hyperpigmentation must also be considered, as it increases the risk of adverse skin reactions.

Failure to diagnose these situations beforehand is a common cause of avoidable complications.
An accurate diagnosis and rigorous assessment are therefore essential before establishing any indication for cryolipolysis.

The crucial role of the protective membrane and calibration

During every cryolipolysis session, a gel-soaked protective membrane is placed between the applicator and skin. This membrane has a dual role : it distributes cold evenly and prevents direct contact between the cold surface and epidermis, reducing burn risks.
The doctor then adjusts every treatment parameter : applicator selection, suction level, exposure duration and continuous temperature monitoring. This technical precision, specific to medical devices and clinical supervision, ensures patient safety at every stage and determines final result quality.

What should you do if an abnormal skin reaction occurs ?

If a patient develops significant pain, blisters, redness persisting beyond 72 hours or any change in skin quality — such as hardening or an open lesion — they should promptly consult the doctor who performed the procedure. Management depends on symptom severity. It may include appropriate local care, such as non-adherent dressings, antiseptics or emollients, together with strict sun protection for several weeks. In some cases, a topical antibiotic or anti-inflammatory treatment may be prescribed, and dermatological follow-up is required for deeper injuries or unusual progression. Rapid intervention is essential to avoid secondary complications such as infection, a pigmented scar or delayed healing.

Can the risk of burns be completely prevented ?

Preventing cryolipolysis-related burns relies on a rigorous medical approach, although zero risk does not exist in medicine. When performed under strictly controlled conditions, this technique offers a high level of safety. This requires a trained doctor who understands the indications, contraindications and technique. A certified medical device with real-time temperature control is also essential. Patient selection must be rigorous, based on thorough consultation and clinical examination, with clear information about warning signs after the session and instructions to follow. Systematic application of the protective membrane and strict adherence to the validated protocol complete this preventive approach. This medical approach, distinct from standardised commercial practices, gives cryolipolysis a particularly favourable benefit-risk balance.

What is the difference between a cold burn and a heat burn ?

The term burn covers very different pathophysiological mechanisms depending on the causative agent. In cryolipolysis, it means a cold burn — also called a frost injury or cold-induced skin injury. Unlike a heat burn, which immediately coagulates tissue proteins, a cold burn acts more insidiously : it induces intense vasoconstriction followed by local ischaemia and gradual disruption of skin structures. Clinically, it appears as pale, sometimes mottled redness with reduced sensation, potentially progressing to swelling or blistering. Initially, this is often less painful than a heat burn, which can delay the patient's awareness of damage. Treatment relies primarily on appropriate moisturising care and close dermatological monitoring to prevent pigmentation or scarring complications.

Article written by Dr Romano Valeria

LinkedIn

dr.romano.valeria@gmail.com

+41 78 209 02 89

["post-28417","infographie","type-infographie","status-publish","hentry

Quels sont les risques de brûlures ou de lésions cutanées avec la cryolipolyse ? - Dr Romano

<meta name="description" content="La cryolipolyse, bien que non invasive, peut présenter des risques rares de brûlures ou de lésions cutanées.
