By blocking acetylcholine release, it temporarily interrupts the nerve signal responsible for sweat secretion in the injected area.
Unlike sympathectomy (which acts centrally and irreversibly), botulinum toxin changes neither overall nerve architecture nor systemic sweat regulation. It acts only on a well-defined area without interfering with sweat glands elsewhere.

Clinical studies on compensatory sweating

Clinical studies over more than 20 years on botulinum toxin for primary axillary hyperhidrosis show an excellent tolerance profile. Compensatory sweating did not occur in the studied cohorts.
When patients report unusual sweating after treatment, it most often reflects increased awareness of areas already sweating normally (back, chest) or confusion between normal sweating and a side effect.
Randomised controlled studies found no significant difference in overall sweating between treated and untreated patients.
Dermatology and neurology experts therefore agree that correctly injected botulinum toxin does not cause compensatory sweating.

Patient feedback : what is actually felt after treatment ?

In practice, patients very rarely report an increase in sweating in other areas. What can occur, however, is a new awareness of the body. When the problematic area stops sweating (often after years of discomfort), the patient becomes more attentive to other bodily sensations, which can create the illusion of sweating elsewhere.
One patient reports: “Since my underarms no longer sweat, I feel like I’m sweating more on my back. But in fact, that was already the case, I just wasn’t paying attention.”
This phenomenon of adjusting body perception is well known in functional medicine. It is not a real transfer, but a sensory re-evaluation, often temporary.

Botulinum toxin and compensatory sweating

Botulinum toxin remains the reference medical treatment for primary axillary hyperhidrosis. Its safety, lasting effectiveness (4 to 6 months) and good tolerance make it widely valued by doctors and patients.
Contrairement à la chirurgie, son effet est réversible. En l’absence de renouvellement, la transpiration revient progressivement à la normale, sans rebond brutal ni déplacement vers d’autres zones.
Compensatory sweating is therefore unjustifiably feared in this context.

Surgical compensatory sweating

Side effects of certain invasive surgical treatments must not be confused with effects observed with non-invasive medical treatments such as botulinum toxin.
Thoracic sympathectomy, for example, treats severe hyperhidrosis, particularly palmar (hand) sweating. It involves cutting or destroying sympathetic nerves in the chest responsible for activating sweat glands in certain regions.
Although this technique may provide lasting relief, it often causes compensatory sweating in a significant proportion of cases (up to 80 % in some studies).
Why does this happen ? Because sympathectomy acts centrally, irreversibly changing a nerve circuit. Unable to activate certain areas to release heat, the body delegates the work elsewhere. This rebalancing is unpredictable, sometimes disproportionate and often very distressing.
Conversely, botulinum toxin has no central action. It temporarily blocks local transmission of the sweat signal rather than interrupting the main nerve pathway. There is therefore no change in overall temperature regulation or redistribution of sweating.

Botulinum toxin does not disrupt body temperature regulation

The human body has between 2 and 4 million sweat glands distributed across the skin. They are controlled by a complex system integrated into the autonomic nervous system, regulating internal temperature according to physiological needs.
When body temperature rises (heat, exertion, emotion), a signal prompts sweat glands to produce sweat, whose evaporation cools the body. This finely adjusted mechanism is distributed across the skin and remains effective even if certain areas temporarily rest.
Blocking sweating in a localised area — such as the armpits — with botulinum toxin therefore inhibits only part of overall sweating capacity. In practice, this concerns less than 3 % of total body surface.
The rest of the body continues regulating temperature without effort or overload. It does not need to compensate because thermal activity remains stable and controlled. Botulinum toxin does not disrupt thermal homeostasis. Patients show neither increased body temperature nor excessive sweating elsewhere, even during intense exertion or very hot weather.
This physiological stability explains excellent treatment tolerance, even after several sessions and in young or athletic patients.

What do official medical recommendations say ?

The absence of botulinum-toxin-induced compensatory sweating is now well established in medical literature. Numerous professional societies have published precise recommendations based on rigorous studies and long-term follow-up.
The International Hyperhidrosis Society, the leading global organisation dedicated to hyperhidrosis management, states in its protocols that “Type A botulinum toxin is a very safe treatment option for primary axillary hyperhidrosis. No significant cases of compensatory sweating have been reported in clinical trials. Side effects are localised, temporary and infrequent.”
The European Academy of Dermatology (EADV) includes botulinum toxin in first-line treatment recommendations for primary axillary hyperhidrosis, emphasising its excellent tolerance profile.
Even health agencies (such as the FDA in the United States) recognise that botulinum toxin treatment causes no systemic disruption and can be repeated without loss of effectiveness or distant complications.
No reputable medical authority therefore lists compensatory sweating as a botulinum toxin side effect. This is now considered a major distinction between medical and surgical approaches.

Article written by Dr Romano Valeria

LinkedIn

dr.romano.valeria@gmail.com

+41 78 209 02 89

["post-28060","infographie","type-infographie","status-publish","hentry

La toxine botulique pour l’hyperhidrose peut entraîner une transpiration compensatoire ? - Dr Romano

<meta name="description" content="La toxine botulique pour l’hyperhidrose agit localement et ne modifie pas la production globale de sueur.
