It causes abundant localised or generalised sweating, often disproportionate to actual temperature-regulation needs. It is unrelated to exercise or high temperatures. It occurs at any time of day, even at rest and sometimes on waking.
Hyperhidrosis is linked to autonomic nervous system dysfunction controlling sweat glands rather than hygiene or emotional sensitivity. In primary forms (the most frequent), this system sends inappropriate signals, causing excess sweat in certain areas while the rest of the body stays dry.
Several solutions exist for this often disabling problem. In localised forms, botulinum toxin has become one of the most effective, best-tolerated and safest treatments.

How botulinum toxin acts on sweat glands

Botulinum toxin blocks release of acetylcholine, a neurotransmitter essential for communication between nerves and sweat glands. This neurotransmitter is the nervous system's signal to trigger sweat secretion.
Injecting a very small amount into the dermis, close to the nerve endings responsible for stimulation, temporarily interrupts this signal. Sweat glands then stop producing sweat in the treated area. The effect is local, reversible, gradual and highly targeted.
Botulinum toxin does not destroy the gland or change sweating elsewhere. It temporarily blocks the mechanism triggering sweat secretion in the overstimulated region.

Which areas can be treated with botulinum toxin ?

Botulinum toxin is particularly indicated where sweating is localised, symmetrical and troublesome in daily life.
Botulinum toxin can regulate sweat-gland activity. However, its recognised medical indication in this field exclusively concerns primary axillary hyperhidrosis, meaning excessive underarm sweating without an identified underlying cause. By temporarily blocking acetylcholine release — the neurotransmitter stimulating sweat secretion — the toxin significantly reduces sweating in this area and provides lasting comfort.
Treatment focuses only on the targeted area without systemic effects.

Does botulinum toxin cause compensatory sweating elsewhere ?

One of the most frequent concerns before botulinum toxin treatment for hyperhidrosis is compensatory sweating. Many patients imagine that stopping sweating in one area — for example the armpits — makes the body sweat more elsewhere, such as the back, thighs or chest. In reality, this reflects frequent confusion between botulinum toxin and sympathetic nervous system surgery.
Compensatory sweating is a well-documented side effect, but mainly after certain operations such as thoracic sympathectomy, which cuts nerves responsible for excessive sweating. These radical procedures sometimes disrupt overall temperature regulation.
Botulinum toxin acts strictly locally without changing central control. Signals are not diverted and other sweat glands are not overstimulated. The rest of the body functions normally. Overall temperature regulation is maintained by untreated areas, which is more than sufficient in most cases.
Clinical studies confirm this : increased sweating elsewhere is extremely rare after botulinum toxin treatment and, when reported, is often subjective, temporary or linked to greater awareness of bodily sensations.
Contrary to a common misconception, botulinum toxin does not create an overall sweating imbalance. It temporarily suppresses a local excess without disrupting the rest of the body.

Toxine botulique et alternatives thérapeutiques pour l’hyperhidrose : pourquoi ce traitement s’est imposé

Hyperhidrosis can be managed through different treatments, but they differ in effectiveness, tolerance and accessibility. Botulinum toxin has gradually become a preferred solution because it offers an excellent balance between safety, results and quality of life.
Topical treatments (aluminium-salt antiperspirants) are generally first-line. Their action is limited, often insufficient for moderate to severe hyperhidrosis and sometimes poorly tolerated (irritation, burns, contact eczema).
Iontophoresis, which passes a weak electrical current through water and skin (mainly used for hands and feet), can work but requires frequent, lengthy sessions. Effectiveness varies considerably between patients.
Systemic treatments such as oral anticholinergics can reduce sweating throughout the body, but side effects (dry mouth, digestive problems, drowsiness) limit prolonged use.
Finally, surgery (sympathectomy), already discussed, remains a last resort, being invasive, irreversible and associated with higher risks, particularly compensatory sweating.
In this context, botulinum toxin offers localised action and predictable effectiveness. It does not cure hyperhidrosis but provides lasting symptom control, radically changing daily life.

Botulinum toxin in aesthetic medicine versus functional medicine

The term botulinum toxin commonly refers to type A, used both aesthetically (for certain dynamic wrinkles) and medically (for neurological indications or primary axillary hyperhidrosis). Although the molecule is the same, clinical approach, doses and goals differ markedly.
For wrinkles, botulinum toxin is injected into specific muscles to temporarily reduce contraction, visibly affecting facial expressions. The goal is aesthetic, the procedure highly localised and doses low.
For hyperhidrosis, injections enter the dermis rather than muscles and target nerve endings responsible for sweat secretion. Doses are higher and the injected surface much larger (for example the entire armpit). No muscular effect or facial change is sought.
In functional medicine, treatment is also guided by objective functional criteria such as the Minor test, reported discomfort severity and mapping of active areas.
It is therefore essential to remember that botulinum toxin for primary axillary hyperhidrosis is a targeted medical procedure rather than an aesthetic one, addressing a precise indication with a specific protocol and clear therapeutic purpose.

A validated, supervised use since the 2000s

Robust clinical studies in the early 2000s validated botulinum toxin effectiveness for primary axillary hyperhidrosis.
From 2004, botulinum toxin use for hyperhidrosis received official health-authority approval (FDA in the United States, then EMA in Europe) for therapeutic use. Since then, dozens of studies have confirmed its value, particularly in dermatology, internal medicine and aesthetic medicine.
More than twenty years of experience has established a rigorous, safe and reproducible protocol with an excellent benefit-risk balance. Indications have expanded, techniques improved and patient satisfaction remained consistent.
Today, botulinum toxin is an integral part of the legitimate, reliable therapeutic options for providing lasting relief to patients with primary axillary hyperhidrosis.

Article written by Dr Romano Valeria

LinkedIn

dr.romano.valeria@gmail.com

+41 78 209 02 89

["post-28054","infographie","type-infographie","status-publish","hentry

Comment toxine botulique agit sur transpiration excessive ? - Dr Romano

<meta name="description" content="La toxine botulique bloque temporairement les signaux nerveux responsables de l’activation des glandes sudoripares.
