It allows the body to release internal heat, particularly during exercise, hot weather or occasional stress. This can affect a specific body area or be more generalised.
In most cases, hyperhidrosis is focal, meaning localised. The most frequently affected areas are the armpits, palms, soles, forehead or scalp and, less commonly, the back or groin.
This localised sweating is often symmetrical : both hands, both armpits or both feet are affected. It occurs independently of outdoor temperature, exercise or clothing. It may appear even at rest, in neutral conditions or cold weather.
This mismatch with the context is one of the most suggestive markers. Sweat is abundant, visible and sometimes dripping. It may soak clothes, cause discomfort when touching, make manual activities difficult or cause unpleasant odours despite excellent hygiene.

Significant social, professional and emotional difficulties

Beyond physical signs, excessive sweating is characterised by its disproportionate psychological and social impact. Those affected often develop anticipatory anxiety : they fear handshakes, avoid light-coloured clothing, restrict movement in transport or public places, or give up certain professional activities.
Discomfort becomes pervasive and sometimes disabling. Some patients report reduced confidence, social withdrawal or signs of anxiety and psychological distress. They do not always dare discuss it with their doctor, believing it is not serious or no solution exists.
This silent distress is one of the most revealing symptoms of hyperhidrosis. It is more than an excessive response to physical discomfort ; it shows that sweating has taken a disproportionate place in daily life, hindering relationships, work or freedom of action.

Often early onset and a chronic course

Another characteristic of excessive sweating is early onset. Primary focal hyperhidrosis (without an identified underlying cause) generally begins in adolescence or early adulthood. It then follows a chronic course, with stable periods and sometimes fluctuations linked to stress, hormonal changes or seasons.
Patients often consult late, sometimes after years of discomfort. This delay reflects difficulty discussing the subject and normalisation of symptoms : many believe they must simply live with it or attribute it to nervousness or personality.
Persistent excessive sweating that does not improve with ordinary antiperspirants and interferes with daily life is not trivial. It deserves rigorous medical assessment because effective treatments are available.

Primary or secondary sweating ?

It is essential to distinguish primary hyperhidrosis, which is idiopathic (with no identified organic cause), from secondary excessive sweating linked to an underlying condition.
When sweating is generalised, begins suddenly in adulthood, accompanies fever, weight loss or fatigue, or occurs at night, a complete medical assessment is required. Hormonal disorders (hyperthyroidism, diabetes), neurological conditions, certain cancers or medicines may cause it.
The doctor plays a fundamental role : identifying the patient's profile, asking about symptom history, examining them and ordering tests if needed. In primary hyperhidrosis, no organic disorder is found. The patient is generally healthy, but the autonomic nervous system regulating sweat is overactive.

Symptoms associated with excessive sweating

Excessive sweating has more than social or psychological consequences. It can also cause secondary skin symptoms, sometimes overlooked but very common. Constant moisture weakens the skin barrier, alters skin pH and encourages certain bacteria or fungi.
Patients may develop chronic irritation, redness, itching or cracks on the fingers or toes. Skin infections such as intertrigo or fungal infections (especially on the feet) are more frequent. Repeated friction from sweat-soaked clothing can also cause constant discomfort, particularly in the armpits or back.
These dermatological symptoms, often perceived as minor, reflect the genuine physical impact of hyperhidrosis. They must be included in overall assessment and guide appropriate care.

Souffrance liée à l’hyperhidrose

It is important to remember that although hyperhidrosis is not life-threatening, it is a genuine, documented, recognised and disabling condition. Symptoms must not be minimised or reduced to temporary discomfort. They are objective, visible and sometimes disabling signs warranting a structured medical approach.

Hyperhidrosis symptoms according to body area

Excessive sweating does not manifest in the same way depending on whether it affects the hands, feet, armpits, or face. These specific locations give rise to particular symptoms, often very telling for the patient, and guide the diagnosis from the first consultation.
In the armpits, one of the most characteristic signs is the rapid appearance of visible sweat marks on clothing, sometimes just minutes after getting dressed. Fabrics become damp, the patient changes shirts several times a day, and avoids certain materials or colors. This location, although common, is often a source of significant social embarrassment, as it is visible whenever the arms are raised or light-colored clothing is worn.
On the hands, symptoms manifest as constant moisture, sometimes streaming, making it difficult to hold objects, use a pen, or handle paper documents. Handshakes are avoided, and professional gestures (care, aesthetics, customer contact) become stressful. This type of hyperhidrosis is very frequently associated with a major psychological impact, often beginning in adolescence.
On the feet, excessive sweating creates a constantly damp environment, causing discomfort in shoes, slipping, a sensation of maceration, and even recurrent fungal infections. This disorder can limit shoe choices, cause odor issues, or restrict sports practice.
On the face, frontal or scalp hyperhidrosis manifests as visible droplets at the slightest effort or stress, streaming down the forehead, sometimes reaching the temples or neck. This makes makeup application difficult, creates persistent shine, and causes discomfort during public appearances, meetings, or social interactions.
These different clinical expressions, although heterogeneous, share a disproportionate and troublesome nature. They are not simply “localized sweating” : they are genuine symptoms, in their own right, of an autonomic disorder that deserves precise evaluation and targeted treatment.

The difference between emotional sweating and true hyperhidrosis

Another fundamental diagnostic distinction is between emotional excessive sweating and primary hyperhidrosis. They look similar but differ in cause, duration and management.
Emotional sweating is a normal physiological reaction to a psychological stimulus : stress, embarrassment or anticipatory anxiety. It is occasional, contextual and often restricted to specific situations (public speaking, an important appointment, social confrontation). It mainly affects the face, hands or armpits and disappears when tension subsides.
Primary hyperhidrosis is chronic and occurs independently of emotional context. Sweating episodes are repetitive, unpredictable, sometimes present at rest or during sleep and unrelated to perceived stress. For no identifiable reason, the autonomic nervous system excessively stimulates sweat glands.
This distinction matters because treatments differ. Emotional sweating may improve with stress management (psychological support, relaxation, cognitive therapies or even low-dose anxiolytics). Hyperhidrosis requires targeted dermatological or aesthetic care : botulinum toxin, iontophoresis, anticholinergics or surgery in extreme cases.
Sometimes both coexist : hyperhidrosis worsens anxiety and anxiety increases sweating. A comprehensive clinical, therapeutic and compassionate approach is therefore important to break the cycle and restore quality of life.

Article written by Dr Romano Valeria

LinkedIn

dr.romano.valeria@gmail.com

+41 78 209 02 89

["post-28052","infographie","type-infographie","status-publish","hentry
