Over time, this area often becomes one of the first to reveal signs of tiredness or ageing. During consultations, patients frequently mention dark circles and tear troughs. This deep ligament marks the transition between the lower eyelid and cheek. When the area loses structural support, a hollow forms, often interpreted as a sign of tiredness or sadness.
Several mechanisms may be involved : bone resorption at the infraorbital rim, loss of malar fat, laxity of the suspensory ligament and deterioration of deep dermal structures (particularly collagen and elastin).
The result is a pronounced under-eye shadow caused by a discontinuity between the eyelid and cheek rather than pigmentation.

Dark circles involve altered colour beneath the eyes and may occur independently of any hollow. Several types can be distinguished :

Pigmented (brown to ochre) : caused by an accumulation of melanin.

Vascular (bluish, purplish) : associated with skin transparency revealing the underlying blood vessels.

Structural or hereditary : a hollow is present from a very young age.

Post-inflammatory : associated with irritation or repeated sun exposure.

Dark circles can exist without tear troughs, and vice versa. They frequently coexist, but distinguishing them remains essential because their underlying mechanisms differ and require specific therapeutic approaches.

What causes dark circles and tear troughs ?

The tear trough results from a deep anatomical aging process, marked by the progressive disorganization of the support structures of the midface. This hollow forms when the malar fat descends, the suspensory ligaments retract, the infraorbital bone resorbs, or, in some young subjects, in cases of underdeveloped cheekbones. It is not merely a superficial defect, but a true volumetric and structural issue, requiring precise morphological analysis for appropriate management.
Dark circles result from a set of well-identified pathophysiological mechanisms. There are mainly three types of dark circles : pigmentary, vascular, and structural. Pigmentary dark circles are due to an excess of melanin in the epidermis or dermis, frequently observed in darker phototypes. Vascular dark circles, on the other hand, are linked to the increased transparency of the periocular skin — particularly thin (on average 0.33 mm) — revealing the underlying venous network, often exacerbated by venous stasis. Finally, hollow dark circles are secondary to under-eye fat loss or resorption of the lower orbital rim, physiological processes related to aging. These three components can coexist and are often influenced by aggravating factors such as lack of sleep, oxidative stress, circulation disorders, or certain dermatological conditions (atopic dermatitis, eczema, etc.). A precise clinical evaluation is essential to adapt the therapeutic strategy.

Can dark circles and tear troughs be treated together ?

Tear troughs and dark circles can certainly be treated together, provided the approach is considered, gradual and tailored to each patient's anatomy. When these concerns coexist — as frequently happens in clinical practice — a combined protocol may be considered, with strict respect for anatomical priorities.
The first step is to correct a pronounced under-eye hollow. Filling the tear trough with hyaluronic acid is often enough on its own to reduce shadowing and noticeably soften the visual appearance of dark circles.
A wait of two to four weeks is then recommended to allow the product to settle and the result to be assessed precisely. If a hollow persists despite restored volume, additional treatment may be offered. Examples include a vascular or pigment laser, a targeted peel or medical skincare designed to improve skin quality and local microcirculation.
This gradual approach, focused on analysing the true causes of dark circles and hollowing, avoids overcorrection and ensures a harmonious, discreet and natural result.

Why do dark circle creams fail to correct tear troughs ?

Many patients spend years trying to conceal or correct tear troughs with over-the-counter cosmetics. Dark circle creams, drainage serums, decongestant patches and so-called "illuminating" skincare are used in the hope of reducing the visible under-eye hollow. Often driven by appealing marketing promises, this approach remains ineffective when genuine anatomical sagging is involved.
Contrary to a widespread belief, tear troughs do not result simply from dehydration or excess superficial pigmentation. They are a structural phenomenon associated with loss of deep tissue support. Over time — or sometimes constitutionally in younger patients with slim faces — malar fat compartments diminish, bone is resorbed at the infraorbital rim, ligaments become lax and skin quality deteriorates. These anatomical changes create a pronounced groove between the lower eyelid and cheek that cosmetics, by definition, cannot correct. No topical treatment, however sophisticated, can restore lost volume or recreate the deep architecture of the middle third of the face.
Certain cosmetic products can indeed temporarily improve the skin's surface appearance : they enhance epidermal hydration, mildly stimulate local microcirculation or reduce visible tiredness after a short night's sleep. However, these effects are superficial and temporary, with no real influence on the underlying structure. Once a tear trough is established, only targeted medical treatment can produce a visible, lasting result.
The reference treatment remains injection of hyaluronic acid specifically formulated to restore volume in this delicate area. Performed precisely, this procedure smooths the transition between the eyelid and cheekbone while respecting natural facial proportions. Very pronounced hollows may require additional techniques such as collagen stimulation through biorevitalisation or photorejuvenation.
These issues must therefore be clarified with the patient during consultation. Many patients arrive with expectations strongly influenced by cosmetic marketing, hoping that simple surface skincare will correct a concern actually involving deep anatomical structures. The doctor has a dual role : to address these unrealistic expectations while offering a personalised treatment strategy based on detailed analysis of facial morphology, tissue quality and facial dynamics. Only a rigorous medical approach respecting each patient's individual anatomy can achieve a natural, harmonious and lasting result.

Who is most at risk of developing dark circles or tear troughs ?

Certain facial shapes, phototypes and genetic backgrounds increase susceptibility to dark circles or tear troughs. This individual variability justifies tailored care that considers anatomy as well as ethnic background, constitutional characteristics and behaviour.

Profiles at risk of dark circles :

Darker phototypes (IV to VI) are more susceptible to melanin-related pigmented dark circles, often affecting both eyes and appearing early.

People with thin, fair skin, particularly phototypes I and II, are more susceptible to vascular dark circles. These often appear purplish and become more visible with tiredness, stress or poor circulation.

Patients genetically predisposed to dark circles often develop persistent under-eye hyperpigmentation. This dark colour, generally stable over time, responds poorly to conventional depigmenting treatments, making management more complex and requiring specific approaches.

Profiles at risk of tear troughs :

Slim, elongated or bony faces : in younger patients, tear troughs may be constitutional. They are often associated with limited cheekbone projection (malar deficiency), accentuating under-eye shadows from early adulthood.

Very athletic patients or those who have lost considerable weight : these groups frequently experience early facial fat loss, particularly in the middle third of the face. This causes volume loss and visible hollowing even in young, healthy patients.

Physiological ageing : with age, gradual resorption of malar and infraorbital bone, combined with laxity of supporting ligaments, inevitably leads to the development or worsening of tear troughs. This process affects all patients but may progress faster in some because of genetic factors or lifestyle.

Recognising these risk profiles at the earliest signs allows an appropriate, personalised prevention strategy to be introduced. It also optimises treatment by precisely targeting the cause of hollowing : volume, bone support or skin quality. Tailored care based on analysis of individual characteristics is essential for a natural, harmonious and lasting result.

Article written by Dr Romano Valeria

LinkedIn

dr.romano.valeria@gmail.com

+41 78 209 02 89

["post-27632","infographie","type-infographie","status-publish","hentry

Quelle est la différence entre la vallée des larmes et les cernes ?
