Excess product, superficial placement or poor technique may cause unattractive results or discomfort.
Treatment must therefore be entrusted exclusively to an experienced doctor trained in this area with complete eye-area anatomical knowledge. In good hands, injections are safe and correctly performed.

What side effects can occur after tear trough injections ?

Certain signs may appear in the hours or days after tear trough injections. They are common and resolve spontaneously :

Redness or small bruises at the injection site.

Slight under-eye tension.

Mild or asymmetric swelling.

These effects are normal and temporary, generally lasting between 2 and 7 days. Healthy habits, cold compresses after injections and adherence to before-and-after instructions help limit them.

Oedema after tear trough injections : causes and treatment

Prolonged oedema is the most frequent tear trough treatment complication and the least understood by patients. Under-eye swelling persists for weeks or sometimes months without infection, creating a puffy appearance.

Several factors can cause this oedema :

Unsuitable hyaluronic acid : some gels attract more water. In a fluid-retention-sensitive area such as the tear trough, they may cause lasting swelling, particularly overnight or on waking.

Overfilling : excess product, even an appropriate hyaluronic acid, saturates tissues, causing interstitial fluid stagnation and permanent puffiness.

Poor technique : superficial or uneven placement can disrupt lymphatic circulation and cause persistent oedema.

Individual predisposition : patients with under-eye bags or slow lymphatic drainage are more susceptible.

Prolonged swelling is not medically dangerous, but it can affect the aesthetic result and disappoint the patient. It is therefore essential to prevent it by adapting the product, technique, and treatment strategy to the patient’s anatomy.

If it persists, there are two options :

Wait : moderate oedema resolves spontaneously within a few weeks.

Treat : substantial or poorly tolerated swelling requires the doctor to consider treatment after clinical examination.

The doctor's role is central : assess predisposition, select the right product and promptly correct side effects when needed.

Tyndall effect after tear trough injections : why does it occur ?

The Tyndall effect is a recognised optical phenomenon producing bluish or grey colour beneath the skin, generally in the tear trough. It follows overly superficial hyaluronic acid placement beneath thin, transparent skin.
Ambient light refracts through the gel, changing perceived colour. This is neither bruising nor inflammation but superficial placement, sometimes compounded by an unsuitable product.
Although purely aesthetic, it may persist if the gel is not naturally absorbed. Once absorbed, later reinjection may be considered in a deeper tissue plane with less hydrophilic acid and technique adapted to periocular anatomy.
This complication highlights the need for expertise. Even small technical inaccuracies become visible here. Experienced doctors prevent Tyndall effect through precision, measured volumes and controlled injection depth.

Overfilling or asymmetry after tear trough injections

As with any filler, mild asymmetry or excess product may occasionally follow tear trough treatment. Thin skin and constant movement make slight imbalance visible, particularly in daylight.
The most frequent causes are :

Natural anatomical asymmetry between facial sides (very common).

Slightly more product injected on one side.

Local fluid retention, often stronger on one side according to lymphatic drainage.

These irregularities are generally mild, temporary and easily corrected. Touch-ups are usually performed after a few weeks once product integration is complete.
Initial caution remains best : slight undercorrection at the first session allows assessment of tolerance, skin response and overall eye-area harmony. A second targeted session can safely refine results.
This two-stage approach ensures natural, symmetrical and lasting results respecting facial expression.

What serious complications can tear trough injections cause ?

Although very rare, serious vascular complications can occur when injected product accidentally enters a vessel. Interrupted blood supply may cause localised skin necrosis or, extremely rarely, visual damage if product reaches a vessel connected to ocular circulation.
These complications do not occur when an experienced doctor trained in orbital anatomy and safety protocols performs injections in a rigorous medical setting. They are principally associated with risky contexts : non-medical injections, untrained doctors or inadequate knowledge of the area.

An experienced doctor takes strict precautions to avoid incidents :

Blunt-tipped cannulas instead of rigid needles to avoid accidentally entering a vessel.

Slow, gradual injection with controlled pressure.

Respect for deep anatomical planes.

Continuous monitoring of tissue response during treatment.

The doctor must also be trained in immediate vascular incident management, with necessary antidotes (particularly hyaluronidase) and responsive medical organisation.
Although exceptional, these complications remain serious. They fully justify restricting tear trough treatment to experienced doctors specifically trained in the area with complete orbital anatomical expertise.

What are the risks of unsuitable hyaluronic acid for tear troughs ?

Hyaluronic acid selection is essential for successful tear trough treatment. This demanding facial area requires precise product choice. An unsuitable gel can cause discomfort or adverse effects.
Available products differ in cross-linking (ability to retain volume), viscosity, concentration and especially hydrophilicity (water attraction). These properties are essential : a gel ideal for cheekbones may be inappropriate beneath the eyes.
Tear trough hyaluronic acid must meet specific criteria :

Low hydrophilicity to avoid fluid-retention oedema.

A flexible formulation respecting natural orbital movement.

Moderate viscosity to integrate without visible or palpable masses.

Overly dense or volumising gels may create artificial prominence or accentuate existing bags. Conversely, overly fluid products may absorb quickly and provide insufficient correction.
Injection depth is crucial. Some gels are unsuitable for superficial skin layers. Thick product placed shallowly increases bluish Tyndall effect risk.
Product longevity must also be understood. An unsuitable gel may remain visible too long, migrate or complicate subsequent correction.
Experienced doctors consider anatomy, depth and desired results when selecting products. This rigour produces natural, safe and harmonious results.

Which technical errors increase tear trough injection risks ?

The injection technique plays a fundamental role in preventing side effects. Even with the best product, a poorly executed procedure can lead to an unaesthetic result, prolonged discomfort, or, in rare cases, a real complication.
Tear trough injection requires millimetric precision. A poorly performed injection can disrupt local microcirculation, cause edema, or result in visible irregularities.

The most frequent technical errors in this area are :

Overly superficial placement : the commonest error may cause Tyndall effect, palpable gel or visible small irregularities.

Excess injected volume : restraint is essential. Generous injections create artificial swelling unlikely to resolve naturally.

Incorrect orientation of the cannula or needle : poor orientation can result in injection into an inappropriate fat compartment or too close to a blood vessel.

No comprehensive facial perspective : tear troughs should not be treated independently. Cheekbone volume loss (malar fat loss) or asymmetry may accentuate hollowing. Correcting only beneath the eyes without restoring midface balance may reinforce contrasts and overall imbalance.

Comprehensive facial care harmonises volume and supports the orbital area for more natural results. Whole-face analysis is therefore essential before injections.

Identical bilateral injections in asymmetric areas : faces are never perfectly symmetrical, and equal doses may create imbalance.

A trained doctor follows fundamental rules :

Use a blunt-tipped cannula rather than a needle in most cases.

Inject very slowly in small quantities.

Place product in the correct anatomical plane, often deeply against bone.

Proceed in stages : initial injections followed by another session if needed.

Even excellent hyaluronic acid may produce poor results with unsuitable technique. Conversely, rigorous, gradual technique respecting structures achieves natural, elegant and safe results.

How can tear trough injection risks be reduced ?

Tear trough injections are technically precise procedures offering natural, discreet and elegant results when correctly performed. Several conditions ensure safety and effectiveness. Every stage, from consultation to follow-up, is fundamental to adverse-effect prevention.

Doctor selection : expertise is the first safety factor. Treatment requires specific eye-area experience and detailed periocular anatomical knowledge. These are not standard injections. Precision, morphological analysis and adaptation to individual anatomy prevent complications. An experienced doctor identifies contraindications (fat bags, poorly drained oedema, bone asymmetry), chooses the best strategy (direct injection, preliminary treatment or refusal) and performs the procedure expertly.

Product quality : hyaluronic acids differ. Tear trough treatment requires minimally hydrophilic gel to limit prolonged oedema, formulated for thin tissues, meeting European medical standards and supplied by manufacturers recognised for safety and traceability. Unsuitable, excessively volumising or cross-linked products greatly increase Tyndall effect, chronic swelling or visible irregularity. Correct selection is decisive.

Injection technique : success rests on controlled technique respecting tissues and anatomical complexity. It must be : slow, allowing appropriate distribution and real-time monitoring ; precise, with complete control of depth and quantity ; gradual, often in two stages : gentle initial correction followed by a possible touch-up after integration. Blunt cannulas are strongly recommended over needles to reduce vascular injury risk. Correct placement with minimal volume achieves results imperceptible to sight and touch.

Follow-up : this is essential to safety. A review around 10 to 15 days checks integration, persistent oedema or asymmetry and permits adjustment or correction. Rigorous follow-up also reassures patients, answers questions and maintains transparent communication throughout.

Article written by Dr Romano Valeria

LinkedIn

dr.romano.valeria@gmail.com

+41 78 209 02 89

["post-27620","infographie","type-infographie","status-publish","hentry

Quels sont les risques de l’injection de la vallée des larmes ?
